Fees for records retrieval will not exceed the rates established by RSMO Section 191.227
PATIENT REQUESTED
Patient may request a copy of their Patient Care Report and Ambulance Billing Record by completing the Release of Medical Records Form, provide photo I.D. and payment fee. Once you have your request ready, please reach out to our Billing Coordinator directly by email, billing@bc-ems.com, or by phone at 816.396.9572.
NON-PATIENT/LAWYERS/THIRD PARTIES
Requests for medical records and/or itemized billing statement on behalf of a law firm, third-party retrieval company, or insurer can be made through the website www.chartswap.com.
ChartSwap is a HIPPA compliant platform launched to facilitate electronic medical and billing record exchange between medical providers and law firms, and other requesting parties. It is free to register and requestors can use ChartSwap to request, track, pay for and download records.
